People in Thurrock will have:
- improved population health and wellbeing outcomes and reduced health inequalities.
- models of care delivery that are flexible, local, offer choice and can be tailored to suit the individual
- access to joined up systems, reduced duplication and services delivered on the evidence of an individual’s needs
- fulfilling lives with the individual and the family at the heart
- preventative services resulting in fewer expensive and intensive interventions (prevent, reduce and delay)
- additional support to play an active role in society for those that need it
- skills and support necessary to achieve aspirations and access good, local jobs.
