Homecare reablement

Homecare Reablement is short-term support for up to 6 weeks that can help you regain the skills and confidence you need to live safely and independently at home.

Who is Reablement for?

Reablement can help people who need support with daily activities because, for different reasons, they are finding it more difficult to do them. The service is for anyone over the age of 18 who is eligible for social care support and would benefit from a period of reablement for up to 6 weeks.

It may not be suitable for people who already have long-term support from a care agency or for people who require specialist support such as palliative care.

The help you will receive

The reablement service can help you to:

  • Find new ways to do things so that you feel safe and more confident.
  • Regain skills and confidence to carry out daily living activities like washing, dressing, and making meals.
  • Look at what else might help (e.g. support to go out, personal alarms, home adaptations or other equipment).
  • Involve your relatives and/or carers in helping you to live more independently – and discuss any support they may need; this may include a Carers Assessment.

What does Reablement involve?

Before starting a programme of reablement, we will discuss what matters to you and learn what is already working well for you, and which areas of your life you may need support with.  If we agree reablement support would be beneficial to you, we will ask you to set realistic goals which our support workers can help you achieve.   Each week, we will discuss the progress you are making and decide whether you need reablement to continue or consider whether you would benefit from a different type of support.

If it is considered reablement support would not be beneficial to you and you agree you will need longer- term support, we will arrange for you to have a social care assessment where we will discuss the support options available to you and what may work best for you and your carers.

What happens after Reablement?

If reablement has worked well, you may find you can manage on your own or only need a little support. If you do need longer-term support, we will arrange for you to have a social care assessment where we will discuss the support options available to you and what may work best for you and your carers.

Will I have to pay for Reablement?

If we agree reablement support would be beneficial to you, there will be no charge for the duration of the reablement support you receive – which could be anything from a few days to a maximum of six weeks.  However, if it is considered reablement support would not be beneficial to you but you need longer-term social care support you may need to contribute to the cost of the care you receive. 

Whether someone contributes to the cost of the care they receive from us will be dependent on their financial circumstances.  If we think you do need social care support, a Customer Finance Officer will help you complete a financial assessment form. The purpose of the financial assessment is to determine whether you are eligible to have some, all, or none of your care and support costs funded by the council. If you are eligible for funding support, we will tell you how much you need to contribute towards your care and support services. The Customer Finance Officer will also offer to check that you are receiving all the benefits that you are entitled to, and help you claim any benefits you may be entitled to.
